Be Prepared for Your Next Doctor Visit

Before you can have a conversation with your doctor, it's important to think about how to describe what you are experiencing. PS or ET movement disorders are complicated and share certain symptoms. Be sure you are aware of what yours are so that you can discuss them with your doctor.

Below you can download a guide to help you discuss your symptoms with your doctor, so that you can communicate more effectively. It is not intended to diagnose a disease.

For each question, circle Yes or No, and share the results with your doctor. If you answer "Yes" to any of the questions, we encourage you to discuss that symptom in detail with your doctor: when it occurs, how often, and its usual intensity.

Caregivers: If your loved one is unable to fill out the self-assessment, please offer to write down the answers for them. Then ask if your loved one would like you to be present when you discuss the answers with the doctor.
